The Hearing Zone (UT), an audiology healthcare provider, reported to HHS OCR on 2014-12-05 that an unencrypted laptop containing ePHI (patient names, dates of birth, and hearing test results) was stolen from a clinic. The breach affected 623 of its own patients and 556 patients from three partner clinics. Police recovered the laptop within weeks. OCR investigation led to media notification, written policies, workforce security training, and encryption of ePHI workstations.
