Business associate Bizmatics, Inc., which provided online storage and management of patient health records, discovered unauthorized access to servers hosting the CE's patient files. Breached ePHI included diagnoses, conditions, medications, and other treatment information, stored on Electronic Medical Records and Network Servers. OCR investigation led to a new BA agreement and documented corrective actions by both parties.
