Broward Health Medical Center (FL) reported to HHS on 2013-10-17 an Unauthorized Access/Disclosure affecting 960 individuals. An employee removed paper patient facesheets from the premises; the documents were subsequently stolen from the employee's home by a visitor. Exposed data included names, dates of birth, addresses, phone numbers, SSNs, insurance information, reasons for visits, employers, and emergency contacts. The employee is no longer employed by the CE. Workforce was retrained. OCR provided technical assistance and obtained assurances of corrective action.
