Bon Secours Mary Immaculate Hospital (part of Bon Secours Health System, VA) reported to HHS on 2013-05-29 that two Certified Nursing Assistants impermissibly accessed the electronic medical records of approximately 5,764 patients over the prior 12 months. Exposed PHI included names, SSNs, dates of birth, addresses, and clinical information. Both employees were terminated, prosecuted, and pled guilty to Federal misdemeanor charges. OCR investigated; credit monitoring was provided to affected individuals.
