Jamaica Hospital Medical Center (NY) reported to HHS on 2014-05-23 an Unauthorized Access/Disclosure affecting 26,162 individuals. From August 1, 2011 through March 27, 2014, five employees impermissibly accessed PHI of Emergency Department patients on Desktop Computers and disclosed it to third parties for solicitation. PHI included names, addresses, DOBs, SSNs, diagnoses, insurance info, and admission dates. OCR provided technical assistance; the CE terminated the employees, redesigned workflows, and committed to a comprehensive risk analysis and access management improvements.
