Lancaster County EMS (Lancaster County, SC) reported to HHS on 2015-06-04 an Improper Disposal breach affecting 50,000 individuals. A safe containing two unencrypted flash drives and two unencrypted hard drives went missing from the CE's administration building. PHI on the missing media included names, addresses, dates of birth, Social Security numbers, medications, medical histories, treatment records, and health insurance information for approximately 55,000 individuals. Breached information located on Other Portable Electronic Device. OCR obtained assurances of corrective actions.
