Linda J White, DDS, PC (a dental practice in VA) reported to HHS on 2016-06-27 an Improper Disposal breach affecting approximately 2,000 individuals. An external hard drive containing a backup of the dental practice's computer server was not returned for proper destruction by an employee. PHI exposed included patients' names, dates of birth, social security numbers, and limited medical information. The hard drive required specific software for access. County officials initiated prosecution of the responsible employee. OCR obtained assurances of corrective action implementation. Breached information located on Other Portable Electronic Device.
