Raymond Mark Turner, M.D. (NV) reported to HHS on 2015-02-26 a Theft affecting 2,153 individuals. One unencrypted laptop was stolen during business hours while the office was updating and encrypting its computers. The stolen laptop contained ePHI including names, addresses, dates of birth, SSNs, driver's license numbers, health insurance information, and medical treatment records. The CE notified HHS, individuals, and media; provided credit monitoring; and implemented an encryption management program. OCR reviewed the risk assessment and provided technical assistance.
