Guardian Pharmacy of Jacksonville (FL) reported to HHS OCR on 2018-03-30 a Hacking/IT Incident affecting 11,521 individuals. An employee fell victim to an email phishing scheme, compromising PHI including names, addresses, dates of birth, treatment and diagnosis information, Social Security numbers, driver's license numbers, insurance information, and financial information.
