VA Caribbean Healthcare System (Puerto Rico) reported to HHS OCR on 2011-05-26 a Theft affecting 6,006 individuals. An employee left documents containing PHI in an unsecured bag at a nursing station. The PHI included names, Social Security numbers, patient care assignments, patient counts, and census lists. Upon discovery, the CE secured the PHI and notified HHS, media, and affected individuals. OCR's investigation resulted in employee discipline/retraining and new ward-rounding procedures. Breached information located on Paper/Films.
