Min Yi, M.D. (CA) reported to HHS on 2014-02-05 a Theft affecting 4,676 individuals. A desktop computer hard drive and a backup external hard drive were stolen from the CE's office. Breached ePHI included names, addresses, phone numbers, insurance IDs, SSNs, checking account information, medical/surgical information, diagnosis and procedure codes, and dates of birth. The CE improved physical safeguards, revised administrative policies, encrypted ePHI, and provided credit monitoring. OCR investigation led to improved HIPAA practices.
