Lee Rice D.O. Medical Corporation d/b/a Lifewellness Institute (CA) reported to HHS on 2016-07-15 a Hacking/IT Incident affecting 2,473 individuals. Malware was installed by cyber-intruders into PrognoCIS, the electronic medical records system operated by business associate Bizmatics, Inc. PHI exposed included full names, addresses, dates of birth, phone numbers, sex, marital status, SSNs, claims information, diagnoses/conditions, lab results, and medications. The BA cooperated with the FBI and engaged a cyber-security firm to assess and remediate. OCR obtained assurances that corrective actions were implemented.
