On July 12, 2018, TMC HealthCare (Tucson, AZ) discovered that paper files containing PHI of 1,776 individuals had inadvertently been left unsecured in an unlocked storage suite. Exposed PHI included names, dates of birth, addresses, medical record numbers, insurance IDs or SSNs, diagnoses, treatment info, medications, and test results. The CE secured the files and notified affected individuals, media, and HHS. OCR obtained corrective-action assurances. Breach submitted to HHS on 09/07/2018. Location: Paper/Films.
