VA Long Beach Healthcare System (CA) reported to HHS on 2018-06-22 an Unauthorized Access/Disclosure affecting 1,030 individuals. During a traffic stop on April 12, 2018, police discovered a CE workforce member had unlawfully taken medical folders containing PHI — names, addresses, dates of birth, SSNs, and clinical information — from patients.
