First Coast Podiatric Surgery and Wound (FL) reported to HHS OCR on 2018-08-27 an Unauthorized Access/Disclosure incident affecting 500 individuals (initial estimate). Breached information located on Network Server. On 2018-08-16, CE discovered its billing software, managed by BA NextGen, had a misconfiguration causing login pages to cross-display between clinics. After investigation, CE concluded no PHI was accessed. OCR obtained assurances that no reportable breach occurred and no individual notification was sent.
