St. Peter's Health Partners (NY) reported to HHS on 2015-01-23 a Theft affecting 5,117 individuals. An employee's unencrypted cellular phone was stolen; the device contained ePHI including names, dates of birth, addresses, phone numbers, and treatment information. The CE notified HHS, affected individuals, and the media. Following the breach, additional technical safeguards were implemented and cellular phone security policies were updated. OCR obtained assurances that corrective actions were completed.
