On April 5, 2015, an employee's laptop was stolen from a vehicle during non-work hours. The laptop contained PHI of 506 individuals including names, addresses, dates of birth, Social Security numbers, and limited treatment information. The device was password protected but unencrypted. Success 4 Kids & Families, Inc. (FL) notified HHS, affected individuals, and media. Remediation included encrypting all computers, upgrading servers, implementing an encryption policy, and staff training. OCR obtained assurances of corrective action completion.
