PHI on network servers included names, addresses, dates of birth, drivers' license and Social Security numbers, diagnoses, conditions, and medications. The CE notified HHS, affected individuals, and the media, and responded by implementing multi-factor authentication and providing additional HIPAA training to its workforce.
