On August 18, 2015, an employee of Barrington Orthopedic Specialists, Ltd. (Barrington, IL) discovered a laptop and an EMG machine were stolen from her vehicle. The devices contained names, dates of birth, and clinical/demographic information for approximately 1,009 individuals. The CE notified HHS, affected individuals, and the media; filed a police report; stopped transporting EMG machines; and retrained the employee on HIPAA policies. OCR reviewed documentation substantiating all response actions. Breach reported to HHS on 2015-09-24.
