On April 14, 2013, a SynerMed / Inland Valleys IPA employee's laptop was stolen from her vehicle at her home. The laptop contained PHI of 3,164 individuals, including names, member IDs, dates of service, reasons for visits, and procedure codes. The laptop was password protected but unencrypted. The CE notified HHS, affected individuals, and the media, and implemented corrective actions including encrypting all computers and improving physical security.
