Dr. Axel Velez (Puerto Rico) reported to HHS on 2011-07-13 a Theft affecting 2,800 individuals. Four desktop computers containing ePHI of 2,143 patients were stolen from the office. Breached data included names, addresses, contact numbers, partial SSNs, dates of birth, diagnostic info, visit dates, patient numbers, referring physicians, and insurance information. The CE improved physical security and received OCR technical assistance on risk analysis and Security Rule compliance.
