Allergy and Asthma Clinic of Fort Worth

Fort Worth Allergy and Asthma Associates (TX, Healthcare Provider) reported to HHS on 2010-08-05 a Theft affecting approximately 25,000 individuals. Several computers, including a server, were stolen in a burglary at the CE's premises. Exposed data included names, addresses, dates of birth, Social Security numbers, driver's license numbers, diagnoses, and conditions. The CE notified individuals, media, and HHS; improved physical security; and adopted an off-site encrypted database. OCR investigation led to amendment of the CE's business associate agreement.
