BHcare, Inc. (CT) reported to HHS OCR on 2012-09-14 a Theft affecting 5,827 individuals. A laptop computer and unencrypted back-up tape containing ePHI were stolen from a workforce member's vehicle. ePHI included names, dates of birth, SSNs, health insurance numbers, and patient assessments/diagnoses. The CE filed a police report, notified affected individuals and media, posted substitute notice, and offered one year of free credit monitoring. OCR's investigation resulted in a risk analysis, employee retraining, and new security policies.
