Alpha-Omega Endocrinology, Diabetes and Metabolism Center, PC (AZ) reported to HHS OCR on 2018-12-12 an Unauthorized Access/Disclosure affecting approximately 1,000 individuals. The covered entity was temporarily locked out of its office when its building was purchased by new owners, creating potential impermissible PHI access across multiple media (desktop computer, EMR, email, network server, paper/films, portable devices). PHI involved: names, dates of birth, addresses, SSNs, claims, clinical and treatment information. The CE determined PHI was unlikely to have been actually viewed or misused. OCR required no further action. The practice subsequently closed.
