Select Benefits Group, LLC dba Dental Select (a HIPAA Business Associate) reported to HHS OCR on 2022-04-26 that an employee fell victim to an email phishing attack, exposing PHI of 1,035 individuals. Compromised data included names, addresses, dates of birth, driver's license numbers, Social Security numbers, claims and financial information, diagnoses, and other treatment information. HHS, affected individuals, and the media were notified. The BA implemented additional administrative and technical safeguards; OCR provided technical assistance regarding HIPAA Security Rule obligations.
