On February 22, 2013, a desktop CPU containing PHI of 1,416 individuals was stolen from The Guidance Center of Westchester (NY). Exposed data included names, dates of birth, addresses, diagnoses, outpatient treatment authorization requests, SSNs, insurance carrier names, and treating physician names. The CE filed a police report, notified multiple state regulators and HHS, offered free credit monitoring, and subsequently encrypted all computers, disabled USB ports, and installed security cameras.
