An unencrypted laptop computer containing the protected health information of 689 individuals was stolen from The Methodist Hospital's unlocked testing office. The information included names, dates of birth, Social Security numbers, age, gender, race, and medication information. Following the breach, the hospital restricted the storage of electronic protected health information to network drives, improved physical safeguards, and retrained employees.
