Pediatric Gastroenterology, Hepatology & Nutrition of Florida, P.A. reported to HHS on 2015-08-24 a Theft affecting 13,000 individuals. An employee removed appointment sheets containing patient names, SSNs, dates of birth, and account numbers without authorization. The Tampa Police Department notified the CE on June 25, 2015 after the documents were found in a criminal investigation. The employee was terminated and criminally investigated. OCR obtained assurances of corrective action.
