The PHI involved included names, addresses, birthdates, drivers’ license numbers, health insurance information, Social Security numbers, financial information, lab results, medications prescribed, diagnoses/conditions, and treatment information. The CE sanctioned the employee involved and retrained its staff. OCR obtained assurances that the CE implemented the corrective actions.
