Centerstone of Indiana, Inc. (IN) reported to HHS on 2022-08-04 a Hacking/IT Incident (email phishing attack) affecting 1,700 individuals. Several employees were subjects of an email phishing attack that compromised PHI including names, addresses, dates of birth, SSNs, claims information, diagnoses and conditions, lab results, and medications. Breached information located on Email. CE notified HHS, affected individuals, and media; implemented additional administrative and technical safeguards. OCR provided technical assistance.
