Heriberto Rodriguez-Ayala, M.D. (TX) reported to HHS on 2010-05-11 a Theft affecting 4,200 individuals. An unencrypted laptop stolen from a personal vehicle contained PHI including names, addresses, phone numbers, dates of birth, Social Security numbers, treatment histories, and driver's license numbers. The covered entity notified affected individuals, HHS, and the media. Following OCR investigation, the practice implemented new policies and procedures, retrained staff, and installed encryption software on all workstations.
