Prima Medical Foundation (CA) reported to HHS OCR on 2016-09-26 a Hacking/IT Incident affecting 2,933 individuals. Business associate Medical Practice Concepts, Inc. experienced a ransomware infection on its network server. Forensic investigation found no evidence PHI was accessed or exfiltrated; however, backup failure during data restoration caused loss of physician-documented records for July 11–26, 2016. OCR consolidated this case into a review of the business associate.
