Cigna Home Delivery Pharmacy (CT) reported to HHS on 2015-11-23 an Unauthorized Access/Disclosure affecting 592 individuals across 13 states. A printing error caused two customer letters to be printed on one sheet of paper (front and back) during a mailing. PHI exposed included names, mailing addresses, and medication information (Paper/Films). The CE notified affected individuals, offered free credit monitoring, and implemented a new letter creation procedure. OCR obtained assurances of corrective actions.
