The PHI involved included names, dates of birth, and treatment information. The CE notified HHS, affected individuals, and the media. In its mitigation efforts, the CE sanctioned the employee, worked with local law enforcement, conducted retraining, and added administrative safeguards. OCR provided technical assistance regarding the HIPAA Rules.
