Clear Dermatology and Aesthetics Center (Brenda LaTowsky, M.D., PLC, dba Clear Dermatology & Aesthetics Center and Investigate MD) reported to HHS on 2019-06-17 a theft affecting 1,282 individuals. The office was burglarized; a flash drive, laptop, and tablet computer containing ePHI were stolen. ePHI included names, addresses, Social Security numbers, financial information, clinical and treatment information. OCR provided HIPAA Security Rule technical assistance. Breached information located on Laptop and Other Portable Electronic Device.
