Vision Care Specialists, P.C. (a part of MyEyeDr Optometry of Colorado, P.C.) reported to HHS on 2020-02-07 a ransomware attack affecting approximately 1,070 individuals. The breached ePHI, located on a Network Server, included names, dates of birth, diagnoses, clinical information, and treatment information. The CE notified HHS, affected individuals, and the media, posted substitute notice on its website, and implemented additional administrative and technical safeguards. OCR obtained assurances that corrective actions were implemented.
