Western Health Screening (MT), a business associate providing onsite blood screenings at hospital health fairs, reported a Theft breach to HHS on 2017-04-14 affecting 15,326 individuals. On February 7, 2017, an employee's car was broken into while en route to a health fair; a portable jump drive containing unencrypted ePHI (names, addresses, zip codes, SSNs) and five encrypted laptops were stolen. Only the unencrypted jump drive resulted in a PHI exposure. Post-breach remediation included employee sanctions, retraining, and encryption of all jump drives. OCR obtained assurances of corrective action implementation.
