Anomalous activity was detected on a single network server supporting clinical trial programs. ePHI exposed included names, addresses, dates of birth, phone numbers, patient ID numbers, and/or SSNs. The CE improved physical security, retrained staff, and OCR obtained assurances of corrective action implementation.
