Lindner Center of Hope (OH) reported to HHS OCR on 2019-09-09 a Hacking/IT Incident (email phishing attack) affecting 3,537 individuals. An employee was victimized via a phishing email, exposing ePHI including names, dates of birth, and Social Security numbers stored in email. The CE notified HHS, individuals, and media; provided credit monitoring; implemented additional technical safeguards; and retrained staff on phishing recognition. OCR obtained corrective action assurances.
