Cedar County Iowa reported to HHS on 2020-10-08 a Hacking/IT Incident (email phishing attack) affecting 1,138 individuals. Employees were victims of a phishing attack compromising ePHI stored in email. Data exposed included names, SSNs, addresses, driver's license numbers, dates of birth, diagnoses, medications, financial data, and other treatment information. The CE notified affected individuals, offered credit monitoring, and implemented additional administrative and technical safeguards. OCR provided technical assistance.
