Visiting Nurse Service Association of Schenectady County reported to HHS OCR on 2010-11-12 a Theft affecting 535 individuals. An encrypted laptop containing ePHI (names, addresses, dates of birth) was stolen from the covered entity. A police report was filed. Following OCR's investigation, the staff member's account was disabled, they were counseled and retrained, and the CE adopted laptop security policies with staff-wide training.
