Alma Aguado MD PA (TX) reported to HHS OCR on 2010-06-21 a Theft affecting 600 individuals. The CE's main server and desktop computers containing ePHI were physically taken from its office. Breached information included patient names, addresses, dates of birth, and social security numbers (stored on Network Server). As a result of OCR investigation, the CE updated its privacy/security policies, retrained staff, and added physical security controls.
