On January 12, 2017, an employee of Saliba's Extended Care Pharmacy inadvertently emailed an attachment containing patient invoices to six current patients. These invoices contained patient names, billing addresses, account balances, and for some, medication names and dosages. The breach, affecting 6,599 individuals, was discovered on January 16, 2017.
