Gair Medical Transcription Services, Inc.

Pinnacle Health Systems reported to HHS on 2010-12-15 that its business associate, Gair Medical Transcription Services, Inc. (PA), suffered a network server compromise that allowed patient medical reports to be viewed online. The breach affected 1,085 individuals. PHI exposed included names, Medicaid ID numbers, dates of birth, and primary physicians. The covered entity terminated its BA relationship, engaged a replacement transcription service, and contracted forensic consultants to remove all exposed records and identify the root cause.
