Arizona Asthma and Allergy Institute (AZ) reported to HHS OCR on 2021-05-03 a ransomware attack affecting 70,372 individuals. PHI stored on a network server was compromised, including names, patient identification numbers, health insurance and claims information, and other treatment information. The CE notified HHS, affected individuals, and the media, and provided substitute notice. Additional administrative, technical, and security safeguards were implemented. OCR provided technical assistance on HIPAA Privacy and Security Rule obligations.
