The hospital reported the theft to HHS on May 3, 2012. The exposed data included names, addresses, Social Security numbers, and insurance information. In response, the hospital implemented enhanced system monitoring, revised employee clearance procedures, installed surveillance cameras, and retrained staff on HIPAA regulations.
