PT Northwest, LLC (OR) reported to HHS on 2015-08-21 an Unauthorized Access/Disclosure affecting 1,500 individuals. Breached information located in Email. The CE inadvertently emailed a patient questionnaire as a CC rather than BCC, exposing up to 1,500 patient email addresses, some of which contained patients' names. The responsible employee was sanctioned. Following OCR investigation, the CE conducted companywide and in-person HIPAA trainings.
