PHI involved included names, addresses, dates of birth, drivers' license numbers, Social Security numbers, diagnoses, health insurance and claims information, financial information, and other treatment information. The CE notified HHS, affected individuals, and media, and provided substitute notice. Mitigation included credit monitoring, enhanced safeguards, and staff retraining. OCR provided technical assistance.
