Thieves stole two encrypted notebook computers containing ePHI (names, addresses, DOB, SSN, medications, treatments). Email cache files were unencrypted. The CE notified law enforcement, HHS, individuals, and media, and posted substitute notice. Remediation included enhanced device security, physical security improvements, policy revisions, and workforce retraining.
