On January 21, 2016, State Farm opened an investigation into employees of a State Farm independent contractor agent in Chino Hills, CA. The investigation found misappropriation of customer funds, misuse of customer financial cards, unauthorized access and modification of customer contact information, and addition of insurance coverage without policyholder knowledge or consent. Affected data included names, contact info, dates of birth, SSNs, driver's license numbers, health insurance info, and financial account/card numbers. State Farm terminated system access for involved individuals and offered 12 months of TransUnion credit monitoring.
